About Us

Grace Home Health is a locally owned home health agency proudly serving North Texas since 2001. For 25 years, our mission has been to provide compassionate, high-quality care that helps patients remain safe, independent, and comfortable in their own homes.

We provide skilled nursing, physical therapy, occupational therapy, speech therapy, medical social work, and home health aide services for patients recovering from illness, surgery, hospitalization, or managing chronic health conditions.

Our experienced clinical team works closely with patients, families, physicians, hospitals, and community partners to provide individualized care and support better outcomes.
